Forme Health is not a provider of medical services. Forme Health operates as a technology platform that connects individuals with independent, licensed clinicians who may provide non-emergency care.

The clinicians available through Forme Health are U.S.-licensed professionals who follow rigorous medical protocols designed to ensure patient safety. They apply clear clinical criteria to determine whether an individual qualifies for treatment with GLP-1 or other prescribed medications. Each assessment submitted through Forme Health is reviewed by a licensed clinician who independently determines eligibility and retains sole discretion over prescribing decisions, including for compounded medications.
